Lipoedema is a condition that can be challenging to diagnose, especially in the early stages. In our Perth practice, we often see patients who have struggled for years to find answers. Diagnosis is based on a detailed medical history and a physical examination, as there are currently no specific clinical tests available. This means a definitive diagnosis is not possible, and a presumptive diagnosis is made based on symptoms and presentation.
You do not need to see a surgeon for a lipoedema diagnosis. For an initial assessment, we recommend visiting a qualified lipoedema therapist. Medicare and Private Health Cover
In Australia, treatment for lipoedema is not eligible for a Medicare rebate, even when symptoms are severe. This applies to patients in Perth and across the country. Private health funds also do not provide rebates for consultations or surgery for true lipoedema.
We are aware that some practitioners use Medicare item number 45585 for liposuction in patients with lipoedema. This item is intended for lymphoedema, not lipoedema, except in rare Stage 4 cases where lymphoedema (lipolymphoedema) is also present. Lymphoedema must be confirmed through assessment and, in some cases, diagnostic imaging such as lymphoscintigraphy. Claiming this item without a confirmed diagnosis of lymphoedema is unlawful, unethical, and not supported by our practice.
Non-Surgical Management
Before considering surgery, many patients explore non-surgical lipoedema treatment options. These may assist in managing symptoms. Approaches include:
- Wearing graduated compression garments
- Manual lymphatic drainage (MLD)
- Regular exercise to support mobility and circulation
- Nutrition plans aimed at reducing inflammation and fluid retention
Some studies suggest that lower-carbohydrate, anti-inflammatory eating plans may assist people who experience pain or difficulty losing weight due to lipoedema. When making dietary changes, it is best to work with a qualified nutritionist to ensure your approach is safe and effective.
Who We Can Offer Surgery To
At our Perth clinic, we only offer surgical treatment for lipoedema to patients who:
- Have been assessed by a lipoedema therapist
- Have a BMI under 30, or a BMI between 30 and 35 with symptoms of pain
BMI and Diagnosis
- Under 30: Diagnosis is generally more straightforward
- 30–35: Diagnosis is possible but more challenging
- Over 35: Accurate diagnosis is extremely difficult
Surgical Treatment
For individuals assessed as suitable candidates, liposuction is one of the surgical options available for managing lipoedema. Water-Assisted Liposuction (WAL) may be considered, as it is generally regarded as gentler on the lymphatic system and may assist in managing symptoms. Outcomes can vary between individuals.
Because lipoedema surgery is classified as cosmetic, there are no Medicare or private health rebates for consultations or surgical treatment.
AHPRA Cosmetic Surgery Requirements
Under AHPRA guidelines, all patients seeking cosmetic surgery must:
- Provide a referral from their general practitioner
- Complete a Cosmetic Surgery Screening Questionnaire before their consultation (accessible via the TouchMD Patient Portal)
Consultation Fee
Our consultation fee for lipoedema surgery is $350, payable in advance to confirm your appointment. Medicare rebates are not available for this fee.
